Acne tends to reach a peak during puberty and before sexual maturity. These changes are most marked on the scalp and around the nose, chin, and cheeks, as well as on the upper chest and back (see Chapter 2). The pilosebaceous gland becomes more differentiated, increases in size, and changes its sebum composition. Undoubtedly, genetic factors also play an important part in determining which girls will suffer and which will not. Although there is no evidence of increased androgen production in men with acne, most women with acne do have increased ovarian androgen production and a reduced SHBG concentration. Acne vulgaris 6 ( Figure 1.4) and hirsutism are never seen in prepubertal children with normal adrenal function, providing further evidence that puberty-related changes trigger these events.

They found that skin penetration and physical stability of flurbiprofen increased 4.5-fold ( Han et al., 2008 González Mira et al., 2011). Nanostructured lipid carriers of flurbiprofen (a nonsteroidal antiinflammatory drug (NSAID) used for therapy of gout, rheumatoid arthritis, and sunburn) were prepared and evaluated by Han and coworkers. Nanostructured lipid carriers of acitretin (used for treatment of acne and psoriasis) were prepared and loaded NLCs show better drug absorption and therapeutic effect for the treatment of psoriasis ( Sue Lee and Koo, 2005 Agrawal et al., 2010). They have reported that two three times increase in drug absorption from the skin was obtained ( Štecová et al., 2007).

Nanostructured lipid carriers of cyproterone acetate (reduces the sebum secretion rate and the acne lesion count) have been prepared by Stecova and coworkers. The topical route is a very interesting area of research for researchers.

Dipanjana Gosh, in Lipid Nanocarriers for Drug Targeting, 2018 1.9.3.6 Nanostructured lipid carriers in topical delivery 20 Tapering the dose or discontinuing acitretin should be considered. 2 Nail disorders, including onychorrhexis, onychoschizia, and periungual pyogenic granuloma, are also more common with high-dose therapy (∼25%–50%) compared with low-dose therapy (0%). Nonscarring alopecia occurs more frequently in high-dose acitretin regimens (∼50%–75% of patients) than low-dose regimens (∼13%) and usually resolves within 6 to 8 weeks of treatment cessation. If symptomatic measures do not help alleviate these side effects, acitretin can be tapered to a lower dose. 12 Symptomatic treatment with emollients and low-strength topical corticosteroid can help relieve mucocutaneous side effects. Retinoid dermatitis resembles unstable psoriasis and can develop in 25% of patients after receiving high-dose acitretin this is a transient reaction that does not require treatment cessation. They are also the most commonly encountered adverse effects and are dose dependent and reversible in nature. Mucocutaneous adverse effects are due to decreased sebum production and impaired skin barrier function (see Table 5.6). Wu MD, in Therapy for Severe Psoriasis, 2016 Mucocutaneous
